Preparing for Your Appointment
Welcome to our office.
In order to provide you with the highest level of care, we ask for your assistance with the following:
Medical Records: Those records related to your present orthopedic condition will be most helpful. Gather all information possible.
Medication List: A list of all current and over the counter medications and vitamins.
Allergies: A list of any known drug, food, and environmental allergies.
Be prepared to discuss your condition: What happened to precipitate your condition? Is it getting worse? How does it limit you from performing your normal daily routines?
Your Questions: We suggest your write down questions you’d like to discuss with your Physician on the day of your appointment.
X-rays / Testing: All x-ray films taken within the last 6 months, including MRIs, Bone Scans or CT Scans.
Dressing for your appointment: Bring or wear shorts without zippers or metal buttons if you are being seen for an injury or conditions below the waist. You may need to have new x-rays taken in our office and therefore, clothing without zippers is ideal. If you do not have shorts, we have disposable paper shorts available.
Insurance and identification cards: We bill your insurance carrier on your behalf. Please bring these documents with you.
