Appointments

Please bring your insurance card and identification card to your appointment.

If this is your first visit to this office, please arrive 15 minutes early to complete forms.

Please call our office at least 24 hours in advance to cancel or reschedule your appointment.

Please notify the office if you require a translator for your visit.

Insurance & Billing

Please bring your insurance card and identification.  If your insurance requires prior authorization for the visit and services, please check with your insurance carrier and/or primary care physician for written authorization for the visit.

Prescription Refills

Please allow 3 business days for prescription refills.  Kindly be advised that narcotics will not be refilled without an appointment.

Referrals/Authorizations

If your insurance plan requires authorization for services such as MRI scanning, physical therapy, surgery, etc, then please allow 10 business days for the referral to be processed.
